E-mail validation After you have sent a completed form, you will receive a confirmation e-mail with a validation link at your address specified during registration. By clicking the link, you will confirm that your address is valid.
Signing a contract at a branch After validating your e-mail, you will receive your login information and the number of your contract from us . To complete your registration, you must personally sign the contract at any Fortuna branch. Czech laws require this.

Login name Please select a user name 5 to 15 characters long. You can use numbers and upper-/lower-case letters without diacritics.	Email address Please enter the e-mail address you use most frequently. We will send you your access details and information about winnings and events at this address.
Phone +420 Please enter your phone number. We will text you the number of your contract and your registration details at this number. You can also have results sent to this number!	Date of birth Please enter your date of birth. If you have turned 18 already, you can place bets with us.
